SalaryThe estimated salary for this role is around $90,000 per year based on industry standards in Southwest Harris County, TX.
About the RoleIn this field-based position as a Behavioral Health Care Coordinator, you'll coordinate the long-term care needs for patients in the local community. You'll work closely with an elite and dynamic team to ensure positive outcomes for patients.
Your key responsibilities will include:
- Assessing, planning, and implementing care strategies tailored to individual patient needs
- Identifying and initiating referrals for social service programs, including financial, psychosocial, community, and state supportive services
- Managing the care plan throughout the continuum of care as a single point of contact
- Communicating with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members
You'll also visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices.

RequirementsTo be successful in this role, you'll need:
- One of the following licenses: RN in the state of Texas, Licensed Professional Counselor (LPC), Licensed Bachelor Social Worker (LBSW), or Licensed Masters Social Worker (LMSW)
- At least 2 years of experience working within the behavioral health setting
- Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook
- Reliable transportation and ability to travel up to 75% of the time in the assigned region
