LPN Healthcare Specialist

The successful candidate will be responsible for delivering high-quality resident services, promoting infection control standards, and collaborating with our interdisciplinary team to achieve exceptional outcomes. Responsibilities:
* Conduct regular wellness visits and identify on-going needs of residents
* Assist Resident Care Director in conducting resident evaluations and data gathering
* Maintain accurate documentation of resident wellness files and communicate effectively with family members and healthcare professionals
* Collaborate with community team members to promote infection control practices and provide clinical support as needed
Requirements:
* Graduate of an approved college or school of nursing
* Current state license as a Professional LPN/LVN
* Minimum of one year experience in home health, assisted living, hospital, or long-term care environment
* Excellent communication and problem-solving skills
* Ability to work weekends, evenings, and flexible hours
