Nursing Care Specialist

We are seeking a dedicated and compassionate Nursing Care Specialist to join our team at Elite Home Health Care.
Key Responsibilities- Conduct initial and reassessment nursing assessments on all clients, adhering to Department of Health guidelines.
- Participate in audits by regulatory bodies.
- Create personalized Plans of Care (POC) that cater to each client's needs.
- Perform a minimum of 23 assessments per week.
- Ensure timely Start of Care visits in Manhattan and the Bronx.
- Travel to various boroughs, including Brooklyn and Queens, as needed.
- Mandatory NYS RN License.
- Prior home care experience is preferred.
- Excellent communication skills.
- Compassionate, caring, and understanding nature.
- Flexibility to accommodate last-minute visit requests.
- Bilingualism, with Spanish-speaking proficiency desired.
The annual salary for this position ranges from $110,000 to $115,000, influenced by factors such as relevant work experience, skills, certifications, and location. We strive to provide our clients with exceptional care while fostering a supportive work environment.
