Senior Clinical Care Specialist

The Ditmas Park Nursing & Rehab is seeking a dedicated Senior Clinical Care Specialist to join our team. This full-time position offers a competitive salary of $65,000 - $80,000 per year.
Job SummaryThis role is responsible for providing basic medical attention to patients and assessing their wellbeing as part of a larger medical team. The successful candidate will have excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
Key Responsibilities:
- Administer medications and nursing procedures to patients as prescribed, ensuring accurate documentation and compliance with regulatory requirements.
- Obtain and record patient temperatures, blood pressures, pulse rates, and other vital signs using standard instruments and procedures.
- Monitor patient symptoms and conditions, identifying responses to medication or changes in health status, and reporting status to registered nurses, physicians, or other licensed healthcare providers.
- Prepare patients for examinations and treatments, maintaining a safe and respectful environment.
- Maintain accurate records of patient conditions and administered treatment, ensuring timely updates and notifications.
- Educate and counsel patients regarding their health, medications, and treatment goals, promoting informed decision-making and empowerment.
- Manage supply inventories, completing requisition forms to ensure adequate stock levels and minimizing waste.
- Supervise unlicensed direct care staff, observing and reporting on their activities to ensure compliance with applicable requirements.
To be considered for this role, candidates must possess:
- A current registration in New York State as a Licensed Practical Nurse (LPN).
- Demonstrated knowledge of nursing processes and procedures, including medication administration and patient assessment.
- Excellent investigative skills, with the ability to identify and address patient concerns and issues.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to write, educate, and work independently, with minimal supervision.
