Job Description:

About the Role

We are seeking a dedicated and compassionate Patient Care Specialist to provide high-quality care to our patients. As a key member of our healthcare team, you will be responsible for delivering patient-centered care, promoting health and wellness, and collaborating with interdisciplinary teams to achieve optimal patient outcomes.

Responsibilities
  1. Deliver Patient Care: Provide direct patient care through nursing activities, including assessing patient needs, developing individualized care plans, and implementing interventions to promote patient health and well-being.
  2. Obtain and Prepare Patient Health Records: Accurately obtain and prepare patient health records for physicians to evaluate, ensuring that all necessary information is included and up-to-date.
  3. Administer Medicines and Intravenous Fluids: Administer medications and intravenous fluids to patients as ordered by physicians, following established protocols and procedures.
  4. Provide Wound Care: Change wound dressings and provide other treatment options to patients, promoting healing and preventing complications.
  5. Monitor Patient Vital Signs: Monitor blood pressures, temperatures, and vital signs at regular intervals, reporting any abnormalities or concerns to physicians and other healthcare professionals.
  6. Operate Diagnostic Equipment: Operate oxygen apparatus, spirometers, and other diagnostic equipment as needed, ensuring accurate and reliable results.
  7. Consult with Supervisors and Physicians: Consult with supervisors and physicians to decide on the best treatment plan for patients, taking into account their unique needs and circumstances.
  8. Direct and Supervise Nurses: Direct and supervise nurses, nurse assistants, and nurse aides, providing guidance and support to ensure that patient care is delivered in accordance with established standards and protocols.
Requirements

To be considered for this role, you must possess:

  • A Bachelor's degree in Nursing (BSN) or an Associate's degree in Nursing (ADN) with applicable work experience.
  • An advanced degree in Nursing (MSN or higher) is preferred but not required.
  • A current registration with the New York State Board of Nursing is essential.
  • Previous long-term care experience is preferable, but we will consider candidates with relevant skills and experience.
  • Good investigative skills, ability to write, educate, and work independently.

Schedule

This full-time position offers an 8-hour shift schedule, Monday to Friday, with opportunities for overtime as needed.
