Responsibilities
  • Conduct regular wellness visits to identify ongoing needs and services of residents.
  • Collaborate with the Resident Care Director to ensure proper follow-through and documentation of resident care services.
  • Assist in conducting resident evaluations and gathering data to inform care decisions.
  • Monitor and follow up with residents who have experienced falls or incidents, documenting and reporting as required.
  • Communicate effectively with residents, families, and healthcare providers to ensure medical needs are met.
  • Maintain accurate and up-to-date resident wellness files.
  • Participate in quality improvement initiatives to enhance resident care and services.
Qualifications
  • Graduate of an approved college or school of nursing.
  • Current state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
  • Minimum of one year experience in a healthcare setting, preferably in long-term care or assisted living.
  • Strong assessment and communication skills.
  • Ability to work independently and as part of a team.
  • Proficiency in computer skills and Microsoft Office applications.
