About the Role

Sunrise Senior Living is seeking a skilled Licensed Practical Nurse to join our team. As a Wellness Nurse, you will play a vital role in ensuring the overall well-being of our residents.

Responsibilities
  • Conduct regular wellness visits for all residents to identify and address any health concerns.
  • Monitor and follow up with residents who have experienced falls, accidents, or incidents, documenting and reporting as required.
  • Collaborate with the Resident Care Director to conduct resident evaluations and gather data on resident health and wellness.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Provide clinical support and assistance to community team members as needed.
  • Stay up-to-date on the latest infection control practices and protocols.
Requirements
  • Graduate of an approved college or school of nursing.
  • Current state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
  • Minimum of one year experience in a healthcare setting, preferably in a long-term care environment.
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a focus on providing exceptional resident care.
