About the Role

We are seeking an experienced Advanced Practice Provider to join our team in Hospice and Palliative Care at Lutheran Medical Center. As a key member of our interdisciplinary team, you will provide high-quality patient care and contribute to the development of our palliative care program.

Responsibilities
  • Collaborate with physicians, nurses, and other healthcare professionals to provide comprehensive care to patients with serious illnesses
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families
  • Provide education and support to patients and their families on end-of-life care and advance care planning
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving patient outcomes
  • Contribute to the development and implementation of policies and procedures related to palliative care
Requirements
  • Master's degree in Nursing or related field
  • Five years of experience as an Advanced Practice Provider in a continuity setting
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to work extended hours and weekends as needed
