Position Overview:

If you reside in New Jersey and are open to traveling up to 50% of your time within your designated area, you will have the flexibility to work remotely while tackling significant challenges.

Key Responsibilities:

  • Conduct outbound calls to evaluate members' current health conditions.
  • Exhibit proficiency in delivering effective telephonic case management that encourages behavioral change for the NJ population.
  • Identify and address gaps or obstacles in treatment plans.
  • Provide educational resources to empower patients in self-management.
  • Collaborate with Medical Directors on complex cases, including participation in rounds.
  • Coordinate care for members effectively.
  • Refer members to external resources as necessary.
  • Facilitate services as required (home health, durable medical equipment, etc.).
  • Educate members on various disease processes.
  • Motivate members to adopt healthier lifestyle choices.
  • Document and monitor findings meticulously.
  • Make follow-up calls to ensure discharged members receive the necessary services and resources.

Required Qualifications:

  • Active, unrestricted RN license in New Jersey.
  • Minimum of 2 years of RN experience in a hospital environment, including acute care and direct patient care, or experience as a Case Manager.
  • At least 1 year of professional experience working directly with elderly or physically disabled individuals in institutional or community settings.
  • 1+ years of experience in long-term care, home health, hospice, public health, or assisted living.
  • Ability to navigate a Windows environment and utilize Outlook, along with proficiency in creating, editing, saving, and sending documents using Microsoft Word and Excel.
  • Demonstrated ability to engage in simultaneous conversation and typing.
  • Access to reliable transportation and willingness to travel up to 50% within the assigned area to meet with employees, members, and providers.

Preferred Qualifications:

  • Bachelor's degree or higher.
  • Certified Case Manager (CCM) credential.
  • Experience with Dual Special Needs Plans (DSNP).
  • Background in working with special needs populations.
  • Familiarity with discharge planning processes.
  • Experience in utilization review, concurrent review, or risk management.
  • Knowledge of government health programs (Medicaid / Medicare).
  • Experience in case management.
  • Background in managed care.
  • Bilingual proficiency in English and Spanish.
  • Strong organizational skills and ability to multitask effectively.

Compensation: The hourly pay range for this position is $28.03 to $54.95, influenced by various factors including education, experience, and certifications.
