Job Summary

The WellFund, LLC is seeking a highly skilled and compassionate Disability Coordinator to join our team. As a Disability Coordinator, you will play a critical role in building working relationships with clients and patients, completing and following the entire Social Security Disability process from initial application to final allowance or denial.

Key Responsibilities
  • Build and maintain relationships with clients and patients to ensure a smooth and successful disability process
  • Complete and follow the entire Social Security Disability process, from initial application to final allowance or denial
  • Identify and resolve barriers to the Social Security Disability allowance while supporting patients and their families
  • Work collaboratively with a team of Patient Advocates, On-Site Managers, Hospital Case Management, and other clinical and non-clinical hospital staff
  • Exhibit superior customer service skills and provide prompt, courteous service to patients at all times
Essential Duties
  • Excellent communication, interpersonal, and telephone skills
  • Maintain and/or create files or record-keeping systems
  • Ensure adequate documentation is maintained, accurate, and secure at all times
  • Collect and review patient information to determine patient's Social Security disability eligibility
  • Prepare application-related documents and review them for accuracy and completeness

Requirements
  • Computer proficiency skills with the ability to learn multiple databases and use Microsoft Office
  • Excellent telephone skills with a comfort level of speaking with claimants during emotionally and physically difficult circumstances using excellent verbal and written communication skills
  • An Associate's Degree or the equivalency is required
  • Experience as a Patient Advocate, Clinical Support Staff, Nursing, Psychiatric Care, Hospital Account Receivables, Health Insurance Benefits, and/or any such experience in the healthcare industry is a plus
  • Considerable knowledge of Medicaid programs, including the Federal Marketplace
  • General knowledge of all agency and community programs and services which could affect the client/applicant and disability process
  • Ability to communicate with clients/applicants, the public at large, and public officials to obtain data, and to explain and interpret rules, regulations, and procedures
  • Ability to perform job functions within structured time frames
  • Must have working knowledge of HIPAA, FDCPA, and Red Flag regulations
Difficulty of Work

The position of Disability Coordinator often includes difficult situations for the patient and their family. This may include terminal illness, traumatic injuries, and situations involving law enforcement. Disability Coordinators are required to ask patients very personal, uncomfortable, and difficult questions regarding their health and history; possibly including criminal acts such as illicit drug use; as well as, detailed descriptions of situations involving patients who are the victims of crime while remaining non-judgmental and compassionate regarding their situation.

Responsibility
  • Interview patients, complete, review, and analyze disability claims throughout the claims process
  • Review the patient's medical, psychiatric, social, and vocational histories in order to complete the Social Security Disability Claim
  • Contact patients/claimants, Patient Advocates, Case Managers, and Social Security Personnel in order to write detailed medical, vocational, and social summaries for each patient/claimant
  • Work in a team concept, but take calls, complete follow-up, interview patients, and build the case file on his/her own
Personal Work Relationships

The Disability Coordinator must deal with a variety of staff levels, conditions, and circumstances throughout numerous hospital departments and government agencies continuously working with colleagues, team leads, supervisors, and management staff.
