POSITION OVERVIEW

The RN Case Manager acts as a vital advocate for patients, assisting them and their families in navigating their healthcare journeys. This role is crucial in the development of personalized care plans, ensuring efficient resource utilization, and establishing safe discharge strategies.

KEY RESPONSIBILITIES

Responsibilities include but are not limited to:

  • Conducting comprehensive assessments of patients to identify clinical, psychosocial, and financial factors impacting care.
  • Collaborating with patients and families to set achievable health goals.
  • Coordinating services among various healthcare providers and community resources.
  • Providing essential tools and information to empower patients in their care decisions.
  • Developing care plans that align with patient needs and available resources.
  • Facilitating effective communication among all stakeholders involved in the patient's care.
  • Identifying community resources to address gaps in care and advocating for patient needs.
  • Ensuring compliance with relevant healthcare regulations and standards.
  • Participating in multidisciplinary rounds to optimize patient care.
  • Educating patients and families about the financial implications of their care options.
  • Tracking resource utilization and addressing any delays in care.
  • Engaging in performance improvement initiatives to enhance service delivery.

QUALIFICATIONS

To be considered for this role, candidates should possess:

  • An Associate Degree in Nursing (BSN preferred).
  • A minimum of one year of experience in case management, discharge planning, or as a Registered Nurse.
  • A valid Registered Nurse license in the state of Florida.
  • Basic Life Support (BLS) certification from an accredited organization.
  • Preferred certification in Case Management.
