Position Overview:

The Recovery Case Coordinator plays a crucial role in the operational success of the department by delivering comprehensive case management services across various units within the organization.

This position involves overseeing client cases with a strong foundation in clinical knowledge and may include providing therapeutic interventions to clients.

The coordinator is responsible for managing discharge and aftercare planning for clients, ensuring they receive the necessary support and resources. Regular communication with referral sources and service providers is essential, along with maintaining an updated resource file. The role also includes participation in quality assurance initiatives within the department.

Qualifications:

  • A minimum of a High School Diploma or equivalent is required; a Bachelor's degree in Social Work, Counseling, or a related field is preferred.
  • At least one year of relevant experience in a behavioral health setting is preferred.
  • Familiarity with the specific patient population served by the organization is advantageous.
  • Experience in case management and/or utilization review is preferred.
  • Certification in CPR and de-escalation techniques is required (training will be provided upon hire).
  • First aid certification may be necessary based on organizational requirements.
Job Functions:

  • Engage in the assessment and formulation of individualized service plans.
  • Design and implement suitable therapeutic activities and programs tailored to client needs.
  • Collaborate with clients and their families to execute effective service plans, discussing available clinical options and their implications.
  • Maintain comprehensive clinical documentation, including records of assessments, service provision, care coordination, and client progress towards recovery.
  • Work in partnership with the clinical team to educate, communicate, and coordinate care among clients, their families, and other healthcare providers.
  • Initiate discharge planning at the time of admission, gathering input from relevant providers.
  • Ensure timely referrals for services outlined in the service plan and coordinate with community resources and service providers.
  • Conduct screenings and assessments for financial entitlements for all clients on the caseload, completing necessary applications.
  • Oversee the development and execution of transition, discharge, and aftercare plans prior to the conclusion of behavioral health services.
  • Document all interactions with referral sources and treatment providers within designated timeframes.
  • Apply appropriate crisis intervention strategies as required.
  • Relay pertinent client information to the treatment team and engage in discussions regarding client goals and motivations.
  • Communicate medical criteria and clinical information between insurance providers and the treatment team as necessary.
