Job Title: Case Manager Coordinator
Job Summary:
KPC Promise Hospital of AMARILLO is seeking a skilled Case Manager Coordinator to join our team. The successful candidate will be responsible for coordinating and supporting day-to-day operations of the Case Management department.
Key Responsibilities:
- Coordinate patient care documents and insurance information
- Maintain accurate patient charts and data
- Prepare necessary reports as directed
- Work in conjunction with the case management team to ensure smooth operations
Requirements:
- High school diploma or equivalent
- Minimum of three years of long-term acute care experience
- Knowledge of Medicare and private payors
- Excellent communication and computer skills
Physical and Mental Requirements:
- Ability to sit for up to 50% of the workday
- Ability to walk, stand, and kneel for up to 15% of the workday
- Ability to lift up to 50lbs
- Ability to work in a fast-paced environment with moderate to high stress levels
