About the Role

We are seeking a compassionate and skilled Certified Nursing Assistant (CNA) to join our Hospice team at our Lake City Care Center. As a CNA, you will play a vital role in providing high-quality care to our patients and their families.

Responsibilities
  • Provide personal care services to patients, including bathing, dressing, and grooming
  • Assist patients with daily living activities, such as feeding, toileting, and ambulation
  • Monitor patients' vital signs and report any changes to the Case Manager
  • Assist with light housekeeping and laundry duties
  • Provide emotional support and companionship to patients and their families
  • Collaborate with the hospice team to develop and implement individualized care plans
Requirements
  • Current CNA certification in the state of Florida
  • Minimum of six months of experience as a CNA
  • Ability to read and follow written instructions and document care given
  • Strong communication and interpersonal skills
  • Ability to work with little direct supervision
