Job Summary

As a RN Case Manager at SSM Health, you will play a critical role in ensuring patients receive the best possible care and support during their hospital stay. This position requires a strong clinical background, excellent communication skills, and a passion for delivering exceptional patient care.

Key Responsibilities
  • Assess patients' discharge planning needs and develop individualized plans to ensure a smooth transition to the next level of care.
  • Collaborate with interdisciplinary teams to identify and address barriers to discharge, including social determinants of health and healthcare access.
  • Develop and implement strategies to reduce readmissions and improve patient outcomes.
  • Provide education and support to patients and families to promote self-management and independence.
  • Participate in quality improvement initiatives to enhance the patient experience and improve clinical outcomes.
Requirements
  • Graduate of an accredited school of nursing with a current RN license in the state of employment.
  • Three years of acute care experience, preferably in a case management or discharge planning role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong analytical and problem-solving skills, with the ability to identify and address complex clinical and social issues.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
