Responsibilities

The Community Engagement Executive is responsible for enrolling eligible patients into the HarmonyCares program. This role will spearhead community outreach initiatives and collaborate to establish relationships with physician offices and healthcare facilities, aiming to engage shared patients into the HarmonyCares program.

Key Duties and Responsibilities
  • Accountable for the community outreach account strategy for designated areas, which may include multiple markets.
  • Conduct comprehensive research on community provider practices and organizational structures to inform account strategies.
  • Analyze and understand patient healthcare needs and utilization to identify effective outreach methods.
  • Engage in direct outreach and in-person visits to physician offices, hospitals, and skilled nursing facilities to educate community providers and enroll patients into the HarmonyCares program.
  • Collect, analyze, and report patient outcomes and experiences to community physicians.
  • Conduct telephonic outreach to eligible patients or caregivers to facilitate enrollment in the HarmonyCares program, supporting team engagement objectives.
  • Possess in-depth knowledge of the program to effectively communicate it to patients, caregivers, and providers while addressing any concerns creatively.
  • Act as the representative of HarmonyCares within the community, collaborating closely with physician leaders to foster community relationships.
  • Utilize electronic tools, spreadsheets, and reports to conduct research, identify innovative strategies, and document and present plans and progress.
  • Meet or exceed established monthly engagement goals.

Qualifications

Required Knowledge, Skills, and Experience
  • College degree or 5 years of progressive career advancement.
  • 3+ years of relevant experience in sales, account management, community outreach, or patient engagement within the medical/healthcare sector.
  • Understanding of value-based care and its impact on patient outcomes.
  • Proficiency in Microsoft Excel, Word, and PowerPoint, with the ability to generate Excel reports.
  • A dynamic and engaging personality with exceptional interpersonal, verbal, and written communication skills.
  • Detail-oriented, with the ability to think critically, respond promptly, and follow up effectively.
  • Compassionate, with a genuine interest in people and a professional demeanor.
  • Curious and committed to continuous learning, with a passion for problem-solving and a proactive attitude.

Preferred Knowledge, Skills, and Experience
  • Bilingual in Spanish, Mandarin, or another language is a significant advantage.
  • Familiarity with CRM and EMR tools is a plus.
