Join Coordinated Care Services Inc as a Care Navigator and play a vital role in transforming the health of our communities. As a Care Navigator, you will be responsible for developing, assessing, and coordinating care management activities to provide quality, cost-effective healthcare outcomes.
Key Responsibilities:- Develop personalized care plans that address the unique needs of our members
- Assess member needs, barriers to care, and resources available to recommend and facilitate the best outcome
- Coordinate between members, families, caregivers, and care providers to ensure timely access to care
- Monitor progress towards care plan goals and collaborate with healthcare providers for revisions or address identified member needs
- Collect, document, and maintain member information and care management activities to ensure compliance with state, federal, and third-party payer regulators
- Bachelor's degree and 2-4 years of related experience
- Graduate from an Accredited School of Nursing if holding clinical licensure
- Equivalent experience acquired through accomplishments of applicable knowledge, duties, scope, and skill reflective of the level of this position
- Competitive pay: $37.25 per hour
