Clinical Patient Care Director

The Clinical Patient Care Manager is responsible for leading the Patient Care Team, ensuring patients and families receive continuity and quality of care. The Team Manager directs all patient care services, collaborating with physicians and acting as a clinical resource for the interdisciplinary team.
Key Responsibilities- Direct all patient care services, including volunteer and bereavement services.
- Collaborate with team physicians and attending physicians as needed.
- Act as a clinical resource for the interdisciplinary team.
- Participate in discharge planning to ensure effective transitions.
- Oversee team schedules and coordinate services for optimal coverage.
- Fill in and provide patient care as needed.
- Staff Supervision and Management
- Interview, select, train, supervise, evaluate, and dismiss team staff.
- Assure staff competence and performance levels through regular field visits and evaluations.
- Act as a resource and mentor for staff on clinical issues, documentation, and customer service.
- Monitor clinical records and team activities to ensure accuracy and compliance with VITAS policy and regulatory bodies.
- Participate in performance improvement activities to address service issues and challenges.
- Comply with QAPI program requirements.
- Assist with surveys and requests from regulatory bodies and intermediaries.
- Resolve service issues with patients, families, and internal customers promptly and satisfactorily.
- Analyze customer service issues on the team to identify causes and implement improvements.
- Participate in potential revocations or service concerns.
- Verify treatments, staffing, and supplies.
- Monitor resources and utilization of clinical operations, including labor, pharmacy, medical supplies, equipment, and mileage.
- Approve all bills and invoices related to patient care services.
- Attend in-services, educational seminars, and workshops.
- Develop and achieve professional growth goals and objectives.
- Participate as a mentor for newly hired Team Managers.
- Reliable transportation with appropriate license and insurance.
- Knowledge of primary medical and nursing care principles, including oncologic nursing and palliative care.
- Basic understanding of interpersonal dynamics within interdisciplinary teams and the process of dying and bereavement.
- Working knowledge of VITAS information system (Vx) and management reports.
- Two years successful supervisory experience in healthcare or equivalent.
- Bachelor's degree in Nursing or related field from an accredited college or university or the international equivalent.
- Current and valid R.N. License to practice in the state where the VITAS program is located.
