Job Summary

LanceSoft is seeking a skilled Travel Nurse RN Case Manager to join our team. As a Case Manager, you will play a critical role in ensuring the highest level of patient care and satisfaction.

Key Responsibilities
  • Develop and implement effective case management plans to optimize patient outcomes
  • Collaborate with healthcare teams to coordinate patient care and services
  • Conduct thorough assessments and evaluations to identify patient needs and develop individualized plans
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
  • Monitor and evaluate patient progress, making adjustments to care plans as needed
  • Participate in quality improvement initiatives to enhance patient care and satisfaction
Requirements
  • 5 years of Director of Case Management experience
  • 2+ years of experience in Case Management (Utilization Review)
  • BLS certification
  • Current licensure in a compact state
  • Covid vaccination required
