Job SummaryThis is an exciting opportunity to work as an integral member of our emergency department care team, providing psychosocial assessments, coordination, and implementation of age-appropriate social medicine services to meet the needs of patients, families, and significant others.
Key Responsibilities- Participate in the ED/CDU care team, providing social medicine expertise in response to care team referrals and case finding of high-need populations.
- Complete psychosocial assessments of patients in ED/CDU, identifying psychosocial needs, developing, and implementing a plan of care, and documenting according to departmental standards.
- Assist patients and their support system with coping strategies related to illness, conduct social medicine consultation and assistance for cases involving abuse, neglect, advance care planning, and legal decision making.
- Provide information and referrals to community resources for post-discharge support, apply knowledge base, theoretical concepts, specific functional skills, and essential social values to effect change in human behavior, emotional responses, and social conditions.
- Contribute to the patient discharge planning process, assessing patient and support system choices, educating them on the risks and benefits of their choices, and facilitating discharge.
- Minimum one year of experience in a healthcare setting.
- Masters degree in Social Work.
- Licensed Clinical Social Worker (Hawaii) or Licensed Social Worker (Hawaii).
- Basic Life Support certification from the American Heart Association.
