Job Summary

Serves as an integral member of the emergency department care team, providing psychosocial assessments and coordination of social medicine services to meet the needs of patients and their families.

Key Responsibilities
  • Participates in the ED care team, providing social medicine expertise in response to care team referrals and case finding of high-need populations.
  • Completes psychosocial assessments of patients, identifies needs, and develops and implements plans of care.
  • Assists patients and their support systems with coping strategies related to illness and provides information and referrals to community resources for post-discharge support.
  • Conducts social medicine consultation and assistance for cases involving abuse, neglect, advance care planning, and legal decision making.
Requirements
  • Minimum one year of experience in a healthcare setting.
  • Masters degree in Social Work.
  • Licensed Clinical Social Worker (Hawaii) or Licensed Social Worker (Hawaii).
  • Basic Life Support certification from the American Heart Association.
