Job Summary

We are seeking a skilled and compassionate RN Home Health to join our team at PeaceHealth. As a key member of our home health care team, you will provide direct skilled nursing care and coordination/management of patients admitted to home care/clinic services.

Responsibilities
  • Perform initial evaluation of patient/home and apply admission criteria to determine if patient is safe candidate for home health/hospice services.
  • Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment.
  • Develop, coordinate, modify and manage written individualized plan of care, as ordered by the primary physician, to meet patient's long and short-term medical goals and appropriate/timely home health outcomes.
  • Coordinate care, collaborate and communicate with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum.
  • Perform discharge planning appropriately towards patient specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.
  • Provide accurate, complete and timely clinical documentation in compliance with regulatory, quality and Agency guidelines.
Requirements
  • Bachelor's Degree in Nursing required; Bachelor of Science in Nursing preferred.
  • 2 years of acute clinical or home health/hospice nursing experience required; 1 year of home health experience preferred or will orient to point of competency.
  • Registered Nurse in state of practice required; Driver's License and proof of auto insurance required.
Working Conditions

This position requires the ability to move around area with frequent sitting, bending/stooping/squatting/reaching/kneeling, and lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently. Exposure to biohazard, body fluids and airborne particles is also required.
