Key Responsibilities- Assessment and Planning: Participate in the assessment, planning, and implementation of clinical transformation activities for assigned services or populations.
- Service Development and Implementation: Support the development, implementation, and evaluation of services and initiatives to improve patient outcomes and satisfaction.
- Collaboration and Communication: Collaborate with management and subject matter experts to understand desired performance, assess learning needs, and develop appropriate training solutions.
- Data Analysis and Reporting: Gather, analyze, and report data related to performance and achievement of project objectives and timelines.
- Process Improvement: Track issues and work to improve processes, efficiency, and compliance of best practice standards, including electronic documentation standards.
- Training and Education: Conduct training and serve as an expert resource for clinical staff.
- Committee and Team Participation: Serve on specialty and/or unit committees, councils, and teams, as well as professional organizations.
- Competency Development: Participate in the identification and development of competencies for clinical staff.
- Interdisciplinary Practice Model: Participate in the development, deployment, and monitoring of the Interdisciplinary Practice Model, safety and quality improvement initiatives.
- Quality Improvement: Support key strategies to drive quality improvement to meet dashboard and strategic objectives, including meaningful use standards.
- Best Practices: Support and facilitate the standardization of best practices across the organization.
- Licensure/Certification: Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
- Education: Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
