Key Responsibilities:
The primary role involves the coordination and management of participants within the Immanuel Pathways PACE program who require hospital admission, short-term rehabilitation, or long-term care. The Nurse Manager acts as an advocate for participants, ensuring the highest quality of care is delivered throughout their healthcare journey.
Qualifications:
A Bachelor’s degree in nursing is preferred, with a requirement for graduation from an accredited nursing program. A valid Registered Nurse (RN) license in the relevant state is mandatory.
Applicants should possess:
- A minimum of two years of nursing experience in acute care, nursing home, or community settings, ideally with a focus on geriatric care.
- At least one year of experience in case management or working collaboratively within a multidisciplinary healthcare team.
- Experience working with elderly or frail populations, or completion of relevant training within the first six months of employment.
- A valid driver’s license and reliable transportation.
