Care Transition Nurse Manager

Job Summary

We are seeking a skilled Care Transition Nurse Manager to join our team at Immanuel. The successful candidate will be responsible for coordinating, facilitating, and transitioning Immanuel Pathways PACE program participants who are admitted to the hospital, in short-term or skilled rehab care, or long-term care.

Key Responsibilities

  • Coordinate and facilitate the transition of PACE program participants to ensure optimal delivery of quality participant care.
  • Act as an advocate for the participant and the PACE program to ensure the best possible outcomes.
  • Work collaboratively with a multidisciplinary team to develop and implement care plans that meet the unique needs of each participant.
  • Provide education and support to participants and their families to ensure a smooth transition.

Requirements

  • Bachelor's degree in nursing or a related field.
  • Current license as a Registered Nurse (RN) in the respective state.
  • Two years of nursing experience in a hospital, acute care setting, nursing home, or community-based setting.
  • One year of experience with case management and/or working with a multidisciplinary team in a healthcare setting.
  • One year of experience working with the frail or elderly population.
