About the Role

We are seeking a skilled clinician to provide care coordination and care management services to individuals in the Transitions to Community Living Program (TCL) Special Needs Populations. As a clinician, you will work with individuals who have complex physical, behavioral health, and social determinant needs, providing whole-person care and addressing their unique challenges.

Key Responsibilities
  • Provide integrated care coordination and care management services to members with identified needs, including behavioral health, physical health, co-occurring, co-morbid, and multi-morbid conditions.
  • Assess members' needs who have transitioned into supportive housing and ensure timely and effective discharge/transition planning and diversion.
  • Perform clinical functions of discharge/transition planning and diversion, including completion of the CMCA and other assessments.
  • Work collaboratively with the care team to provide clinical consultation and support.
Requirements
  • Master's degree in a human services field, preferably with a focus on mental health or substance abuse.
  • Minimum of one year of post-Master's experience in a clinical setting with the MH/SU population.
  • Fully licensed by the state of N.C. as a LCSW, LPA, LCMHC, LMFT, or LCAS.
  • Valid driver's license and residency in North Carolina for remote status.
