**Job Summary**

We are seeking a skilled Care Coordinator to join our team at Nemours Foundation. As a Care Coordinator, you will play a critical role in ensuring the seamless coordination of care for our patients.

**Key Responsibilities**

* Collaborate with the interdisciplinary team to assess patient needs and develop individualized care plans
* Coordinate with healthcare providers to ensure timely and effective transitions of care
* Identify and address potential barriers to care, such as social determinants of health

**Standards of Practice**

**Requirements**

* Bachelor's degree in Nursing or related field

* Strong communication and interpersonal skills
