Responsible for delivering nursing care under the direct oversight of Physicians and Advanced Practice Nurse Practitioners (APRNs).
Effectively coordinates patient transitions throughout the medical facility, ensuring prompt rooming, turnover, and patient flow to enhance physician and APRN productivity.
DUTIES AND ESSENTIAL FUNCTIONSOverall performance evaluation based on the following essential job responsibilities:
Prepares patients for physician and/or APRN consultations by consistently obtaining vital signs, chief complaints, medical history, medication updates, and health maintenance records (e.g., last mammogram) during each visit, ensuring that best practice alerts are addressed prior to the consultation.
Collects relevant health information through physical assessments and biological samples. Gathers specimens for laboratory evaluation.Provides fundamental patient care as directed, adhering to practice guidelines, standards of care, and applicable federal/state regulations.
Documents assessments, effectiveness, and timeliness of care/interventions.
Executes and/or assists in prescribed treatments and medical procedures, administers prescribed medications, and monitors and records treatment progress and patient responses.
Employs organization-wide safe medication practices, including, but not limited to, the five rights of medication administration, barcode scanning, and other medication safety protocols specific to the unit/department.
Possesses knowledge of actions, usual dosages, indications, and side effects of administered medications.Actively manages nursing responsibilities in a timely manner and performs registration tasks as needed, including callbacks, referrals, and customer check-in/out, as evidenced by positive customer feedback.
Provides educational materials and teaching to patients and/or significant others as necessary, according to care, treatment services, and identified risks specific to the patient, documenting patient and/or family comprehension.
The Lead LPN performs the aforementioned duties while also guiding the daily operations of the team.EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
Minimum requirement:
Graduation from an accredited Practical Nursing program.
Maintains an active LPN license in the Commonwealth of Kentucky (or other eNLC state with multistate privileges) and/or Ohio as required by department and potential assignment.
Achieves and maintains American Heart Association Basic Life Support (BLS) certification.Preferred qualifications:
One year of LPN experience.
WORKING ENVIRONMENT
Constantly works near moving mechanical parts and is regularly exposed to blood and/or body fluids and infection.
Occasionally exposed to toxic or caustic chemicals and risk of radiation. The noise level in the work environment is typically moderate.
PHYSICAL DEMANDSOccasionally required to remain in a stationary position (standing or sitting). Frequently required to walk and move about.
Frequently required to operate a computer and other equipment.
Frequently required to use fine motor skills to handle and operate instruments/devices.
Constantly required to lift and/or move items up to 10 pounds.
Frequently required to lift and/or move items up to 50 pounds.
Occasionally required to lift and/or move items up to 100 pounds.
Constantly required to communicate with patients and other care team members, necessitating talking and hearing.
Specific vision abilities required for this role include close vision, distance vision, color vision, peripheral vision, and depth perception.
Occasionally required to position oneself to meet patient needs.
