Our clinicians work collaboratively with primary care physicians, case managers, and other healthcare professionals to deliver high-quality care and support patients in achieving their health goals.

The Role
As a Home Risk Assessment Clinician, you will conduct thorough assessments of patients' medical history, medication adherence, social environment, and other risk factors. This critical information will be used to identify potential care gaps and develop personalized plans to address these needs. You will work independently, but as part of a larger team, to ensure that each patient receives seamless and effective care.

Responsibilities
- Conduct thorough adult and geriatric assessments, including medical history, diagnosis, and treatment planning
- Collaborate with primary care physicians, case managers, and other healthcare professionals to develop and implement individualized care plans
- Provide education and support to patients and their families to promote healthy behaviors and outcomes
- Utilize electronic health records to document assessments, care plans, and other relevant information

Requirements
- Master's degree in nursing or a related field, or equivalent experience and satisfactory completion of NP licensure
- Current RN and NP licensure in state of practice, including prescription authority or ability to obtain prescriptive authority
- Board certification by AANP, ANCC, or AACN in a Matrix-approved specialty
- 1+ year of experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health record software
- Excellent verbal and written communication skills
