About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to fill the position of MDS Coordinator at our Lebanon Center in Lebanon, NH. As an LPN MDS Coordinator, you will play a vital role in the Resident Assessment Process, ensuring the delivery of high-quality patient care and nursing center reimbursement.

Key Responsibilities
  • Direct the nursing center interdisciplinary Resident Assessment Instrument (RAI) process, adhering to guidelines for completion, computer encoding, and electronic State transmission of the MDS.
  • Participate in developing individualized resident Care Plans, identifying resident needs and projected outcomes, as required by Federal and State regulations.
  • Train new members of the interdisciplinary team on clinical content and electronic completion of applicable MDS items.
Requirements
  • Must be an LPN in good standing and currently licensed by the state.
  • Previous experience in a hospital, long-term facility, or other healthcare-related facility is required.
  • Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures, as well as laws, regulations, and guidelines governing long-term care.
  • Must have computer skills, including Windows applications.
  • AccuMed experience is preferred.
  • Previous experience as an MDS Coordinator is preferred.
  • Prior experience in completing MDSs and Care Plans is required.
  • Demonstrated proficiency in RAI/PPS processes is required.
  • Must maintain current BLS/CPR certificate (excluding ALFs/ILFs).
