Responsibilities:

  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Assess patient needs and develop individualized care plans to meet those needs.
  • Document patient progress and communicate with healthcare teams to ensure seamless care.
  • Work collaboratively with patients, families, and caregivers to promote patient-centered care.
Requirements:
  • Current, unrestricted LPN license in the state of practice.
  • Minimum six months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience preferred.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
