LPN Case Manager Job Description

Elderwood is seeking a dedicated and compassionate LPN Case Manager to join our team. As a key member of our nursing staff, you will play a vital role in ensuring the health and well-being of our residents.

Responsibilities:
  • Supervise and administer medications to assigned residents, monitoring those participating in self-medication programs.
  • Monitor personal care provided by Resident Assistants and assist as needed.
  • Report changes in condition, daily needs, and progress of residents to the Director of Nursing.
  • Follow the plan of care for each resident and maintain documentation in Case Notes, Medication Administration Records, and other required records.
  • Assist in problem-solving related to staffing, supplies, and resident care during shifts.
  • Ensure resident care standards are maintained and residents' rights are respected.
Qualifications:
  • Credentials as a Licensed Practical Nurse with a current State license required.
  • Experience in geriatric nursing preferred.
  • Organizational skills and ability to supervise department staff effectively required.
