Job Summary

We are seeking a skilled Nurse Case Manager to join our team at Elevance Health. As a key member of our care management team, you will play a critical role in ensuring our members receive high-quality, patient-centered care.

Key Responsibilities
  • Assess and develop individualized care plans to address the unique needs of our members.
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to optimize member outcomes.
  • Communicate with members, families, and healthcare providers to ensure informed decision-making and timely interventions.
  • Participate in multidisciplinary teams to develop and implement care management strategies.
Requirements
  • Current, unrestricted RN license in applicable state(s) required.
  • Minimum of 3 years of clinical experience in a healthcare setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work in a fast-paced environment, prioritizing multiple tasks and deadlines.
Preferred Qualifications
  • Certification as a Case Manager (CCM) or other relevant certifications.
  • Experience working in a care management or case management role.
  • Knowledge of healthcare regulations, policies, and procedures.
