About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team at Adoration Home Health and Hospice. As a Registered Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals and families in their homes and communities.

Responsibilities
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide professional nursing care, including administering medications, treatments, and performing various medical procedures.
  • Collaborate with the interdisciplinary team to coordinate patient care and ensure seamless transitions between healthcare settings.
  • Communicate effectively with patients, families, and healthcare professionals to ensure that patient needs are met and that care is delivered in a timely and efficient manner.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote high-quality patient care.
Qualifications
  • Graduate of an accredited school of nursing with a current Registered Nurse licensure in the state.
  • One year of hospice experience preferred, but not required.
  • Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.
  • Nursing skills per competency checklist.
