About the Role

We are seeking a skilled Registered Nurse to join our Admissions team at Heartland Hospice. As a key member of our team, you will be responsible for conducting admissions for weekend referrals, ensuring that patients and families receive the highest level of care and support.

Key Responsibilities
  • Conduct admissions for weekend referrals, assessing patients and families to determine the best course of care.
  • Utilize the nursing process to contribute to each patient's total plan of care, working closely with the Admissions Manager and other healthcare professionals.
  • Visit patients and families in their place of residence or in the hospital to assess for appropriateness, explain services, monitor condition, or admit to hospice services in accordance with the admission procedure.
  • Participate in evening/weekend on-call as required, providing 24/7 support to patients and families.
Requirements
  • Registered Nurse license in the state where the program is located.
  • Bachelor's Degree and C.H.P.N. certification preferred.
  • Minimum of 1 year general nursing experience, Hospice experience preferred.
  • Minimum of 2 years Hospice Health Care or Medical Office experience, preferred.
  • CPR Certification.
