Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Providence Sacred Heart Medical Center in Spokane, WA. As a Clinical Pharmacist - Transitions of Care Specialist, you will play a critical role in ensuring the safe and effective transition of patients from one level of care to another.
Your primary responsibility will be to oversee the admission medication reconciliation process for our hospitalized patients. You will work closely with our healthcare team to identify and mitigate potential medication-related issues, ensuring that our patients receive the highest quality care.

Key Responsibilities:
- Oversee admission medication reconciliation for hospitalized patients
- Collaborate with healthcare team to identify and mitigate medication-related issues
- Develop and implement quality improvement initiatives to enhance patient care
- Precept students and residents in clinical settings
- Contribute to leadership development and growth opportunities
Requirements:
- PharmD or Bachelor's Degree in Pharmacy
- Washington Pharmacist License (when eligible)
- Basic Life Support-Provider (BLS) certification (where applicable)
- Acute care hospital pharmacy experience (1 year or equivalent)
