Job Overview

The Area Agency on Aging & Disabilities of SW WA is seeking a dedicated Registered Nurse to join our team as a Senior RN Care Consultant. This role involves providing comprehensive care to adults with disabilities and older persons in their homes, promoting independence and quality of life.

Job Responsibilities
  • Care Coordination: Assess client needs, coordinate medically related referrals, and review the performance of care providers.
  • Nursing Assessment: Conduct thorough assessments, identify areas for improvement, and develop personalized care plans.
  • Client Education: Provide education and support to clients and their families to promote self-care and independence.
  • Team Collaboration: Work closely with healthcare professionals, case managers, and other stakeholders to ensure seamless care delivery.
Requirements
  • Bachelor's Degree in behavioral or health sciences
  • Current license to practice as a Registered Nurse in the State of Washington
  • At least three years of nursing experience, including one year in a community setting
  • Valid driver's license and reliable transportation
Benefits

We offer a competitive salary range of $85,000 - $105,000 per year, depending on experience, plus medical, dental, and vision insurance, generous sick and vacation leave, and WA state PERS retirement benefits.
