Job Summary:

We are seeking a highly skilled Licensed Vocational Nurse to join our team at Del Monte Village Assisted Living and Memory Care in Pacific Grove, CA. As a Case Management LVN, you will be responsible for providing exceptional care to our senior residents, working closely with local housing authorities and VA Veteran Affairs teams to ensure the best possible outcomes.

Key Responsibilities:

* Develop and implement individual care plans for up to 12 residents
* Routinely monitor and assess residents for mental health and welfare
* Collaborate with local agencies to provide comprehensive support services
* Work independently to manage caseload and prioritize tasks

Requirements:

* LVN license required
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Compassionate and caring approach to resident care
