Position Overview

We are actively seeking a Client Care Coordinator within our Clinical department. This role involves delivering comprehensive case management services, which include evaluating the health and long-term care requirements of each participant.

Key Responsibilities

  • Serve as the team leader in crafting and executing the consumer's care strategy, including the authorization of service scope, duration, and amount.
  • Possess a Bachelor's degree in Social Work, Psychology, Sociology, Counseling, Gerontology, or Nursing, or have one year of direct service experience with elderly individuals or those with disabilities.
  • Demonstrate experience in conducting assessments, developing service plans, and monitoring progress.
  • Exhibit flexibility in scheduling to accommodate family needs, including having reliable transportation, a valid driver's license, and car insurance as mandated by state law, along with the ability to travel.
  • Adhere to the CareStar principles in all job functions.
  • Understand and comply with CareStar's Policies and Procedures.
  • Maintain confidentiality and treat any unauthorized disclosure of information as a serious violation.
  • Follow the Acceptable Use Policy when utilizing any information systems managed by CareStar, Inc.
