Job Summary

As a Nurse Care Coordinator at Prisma Health, you will play a vital role in providing direct patient care, education, and support to patients in our fellowship-trained specialty clinic setting.

Key Responsibilities:
  • Direct Patient Care: Provides high-quality, patient-centered care in an outpatient specialty setting, maintaining advanced knowledge of diagnoses, medications, and treatment options specific to the specialty.
  • Phone Triage: Serves as the primary contact for patient/family members, performing effective phone triage and involving physicians when needed, utilizing critical thinking skills and advanced knowledge to handle the majority of concerns.
  • Scheduling and Coordination: Triages current patient population and incoming referrals, prioritizing scheduling of office visits and testing, and working with physicians to determine scheduling of emergent patients.
  • Procedural Support: Assists with in-office procedures, providing comfort and safety measures for patients before, during, and after the procedure.
  • Patient Education: Identifies learning needs of patients/family members and works with team members and community resources to ensure objectives are met, developing and maintaining educational tools to promote compliance with treatment plans.
  • Patient Safety: Assumes responsibility for patient safety by maintaining a safe environment and completing environmental rounds, enforcing patient care standards.
  • Staff Development: Champions the development and implementation of the orientation program, ensuring new staff are educated and prepared to provide safe, effective care, and may define competencies for the specialty and observe and document staff proficiency on an annual basis.
  • Community Outreach: Works with team members to plan, develop, and implement educational programs and seminars in the community and within Prisma Health, acting as a resource for other team members, hospital staff, and specialty practices.
  • Professional Development: Evaluates own nursing practice in relation to professional practice standards, guidelines, state statues, policies, procedures, rules, and regulations, and maintains competency through education and professional activities.
Requirements:
  • Education: Associate's Degree in Nursing
  • Experience: 1 year of clinical nursing experience within the specialty
  • Certifications: RN - Registered Nurse
