Job Title: Admissions and Care Planning Professional | Full-Time

CareSpring Health Care Management is seeking an experienced Admissions and Care Planning Professional to join our team in Loveland, OH. The successful candidate will be responsible for assessing new and existing patients to ensure their care needs are addressed and documented on the Minimum Data Set (MDS).

Estimated Salary: $32.25 - $41.75 Per Hour

Job Summary

The Admissions and Care Planning Professional will play a critical role in ensuring that our residents receive the highest level of care possible. This includes assessing new and existing patients, documenting care needs on the MDS, and working collaboratively with our interdisciplinary team to develop and implement effective care plans.

Responsibilities

The Admissions and Care Planning Professional will be responsible for:

  • Assessing new and existing patients to identify care needs and develop effective care plans
  • Documenting care needs on the MDS and ensuring accurate and timely submissions
  • Collaborating with the interdisciplinary team to develop and implement care plans
  • Monitoring resident progress and making adjustments to care plans as needed
  • Ensuring compliance with regulatory requirements and industry standards
  • Participating in ongoing education and training to stay up-to-date on best practices and industry developments

Requirements

  • A current LPN or RN license in the state of Ohio
  • A minimum of 2 years of experience in a similar role
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Flexibility to work varied shifts, including weekends and holidays
