Job Title: Licensed Practical Nurse/LPN

Job Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Avamere at Cascadia Village. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Responsibilities:
  • Complete accurate and thorough record-keeping for resident admissions, transfers, and discharges.
  • Coordinate with nursing staff and support personnel to plan daily services, programs, and activities that meet resident needs.
  • Administer medications and review medication cards for completeness and accuracy.
  • Review resident care plans to ensure goals, problems, approaches, and revisions are aligned with nursing needs.
  • Develop and implement programs, goals, objectives, policies, and procedures to provide quality care.
  • Provide leadership and guidance to nursing personnel, ensuring direct care is delivered in accordance with resident care plans and wishes.
  • Develop and maintain positive relationships with nursing staff, department personnel, residents, family members, and other stakeholders involved in resident care.
  • Participate in facility surveys by authorized government agencies.
Qualifications:
  • Must possess a nursing degree from an accredited college or university.
  • Must have a current, unencumbered, active license to practice as an LPN in this state.
  • Knowledgeable of nursing and medical practices, procedures, laws, regulations, and guidelines related to nursing care facilities.
  • Experience with Electronic Medical Records and computer documentation systems.
  • Ability to provide proof of COVID-19 vaccination or qualifying exemption.
