Pay: $45/hr + reimbursed expenses

Position Type: Full-time, Remote

Summary: Morgan Stephens is seeking a skilled Remote Case Manager to join our team. As a key member of our healthcare team, you will be responsible for managing and coordinating the care of Managed Care members in the Ft. Myers area. This role involves regular local travel to assisted living facilities, nursing homes, and members' homes for face-to-face visits.

Key Responsibilities:

  • Conduct face-to-face visits with members to assess their healthcare needs and develop personalized care plans.
  • Collaborate with members, caregivers, and healthcare providers to ensure seamless transitions between healthcare settings.
  • Identify gaps in care and access resources to address them, ensuring optimal clinical, financial, and quality of life outcomes.
  • Provide emotional support and guidance to caregivers and members, connecting them with community resources and social services programs as needed.
  • Arrange transportation and additional services, such as physical therapy or meal delivery, in collaboration with primary care providers and other stakeholders.

Requirements:

  • Bachelor's degree in a related field, such as social work or health administration.

Work Hours: 8:30 AM - 5:00 PM
