Job Title: Care Navigator

Broward House, Inc. is a not-for-profit community-based organization serving Broward County with a focus on HIV care and Prevention, housing, and behavioral health. We are seeking a highly skilled Care Navigator to join our team.

Job Summary:

The Care Navigator will be responsible for coordinating and navigating medical and social services to support community individuals in care access and adherence based on their resources. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with high-risk, vulnerable populations.

Key Responsibilities:
  • Engage, counsel, manage, and effectively work with high-risk, vulnerable patient populations, including chemically dependent persons.
  • Engage, counsel, manage, and effectively work with persons in crisis.
  • Locate individuals requiring health care that are not currently accessing or adhering to services.
  • Complete assessments of clients' strengths, needs, abilities, and preferences.
  • Develop individual plans of care and/or transition plans based on identified clients' needs.
  • Provide direct support to clients in implementing activities to meet case plan goals and objectives.
  • Arrange for services through linkage, referral, and navigation services.
  • Provide HIV, STD, Medical Adherence, and Risk Reduction Education to individuals.
  • Guide clients in applying and accessing eligible benefits.
  • Prepare and facilitate groups, including life skills, social rehab, and psycho-educational sessions.
  • Support clients in communication with external and internal providers.
  • Meet with internal and external interdisciplinary treatment team members to coordinate client services and document outcomes of meetings.
  • Submit all documentation in an accurate and timely manner.
Requirements:
  • Two years of experience in Social Services or equivalent experience.
  • Experience personally navigating the system of care.
  • Knowledge of HIV/AIDS and conducting Intervention/Prevention services with minority and at-risk populations preferred.
  • Effective verbal and written communication skills.
  • Solution-focused approach to conflict and challenges.
  • Ability to effectively engage and work in a stressful environment with high-risk, vulnerable client populations and persons in crisis.
  • Ability to establish and maintain effective working relationships with employees, collaborating agencies, and the public.
  • Ability to demonstrate and model culturally competent behavior.
  • Ability to effectively communicate with people at all levels and from various backgrounds.
  • Ability to maintain a strong sense of respect for confidentiality involving both clients and fellow employees.
  • Moderate computer skills.
  • Ability to work independently with minimal direction.
  • Basic office equipment operation skills.
  • Knowledge of Substance Abuse and Mental Health Symptoms and basic interventions.
  • Ability to legally operate a motor vehicle.
