As a Peer Navigator, you will play a vital role in supporting individuals with behavioral health challenges, helping them navigate the healthcare system, and connecting them with community resources.

Job Summary:

The Peer Navigator position requires a strong understanding of behavioral health challenges, excellent communication skills, and the ability to build rapport with individuals from diverse backgrounds. You will work collaboratively with our clinical and non-clinical staff to provide non-clinical supportive services, including education, advocacy, and assistance with accessing healthcare services.

Key Responsibilities:
  • Build relationships with patients and the interdisciplinary treatment team to support continuity of care.
  • Educate patients and staff about your role as a Peer Navigator.
  • Work with patients one-on-one to address discharge planning and inform them of community resources.
  • Support patients with adherence to treatment recommendations.
  • Maintain all required paperwork and documentation in a timely manner.
  • Develop and maintain relationships with community-based treatment agencies and human service providers.
Requirements:
  • Peer Navigator position requires lived/personal experience with behavioral health challenges.
  • Associate or bachelor's degree in social work, psychology, human services, or related field preferred but not required.
  • 1+ years' experience is preferred.
  • Certified Peer Specialist Certification (or certification within the first year of hire).
  • Must be a reliable self-starter with the ability to work both independently and as part of an interdisciplinary team.
