At Northern Rivers Family of Services, we are seeking a dedicated and compassionate Community Service Provider to join our team. As a Community Service Provider, you will play a vital role in providing home and community-based services to children with emotional and mental health challenges.

Job Summary:

As a Community Service Provider, you will work closely with children and families to identify goals and objectives of treatment, provide services based on the needs as identified in the Treatment Plan, and coordinate with the child and caregiver on the scheduling and provision of services. You will also participate in relevant treatment meetings and attend all required trainings.

Responsibilities:
  • Provide services within a Family-driven Care framework, respecting family choice and voice, and utilizing strategies that empower families.
  • Work with family members to identify goals and objectives of treatment.
  • Provide services based on the needs as identified in the Treatment Plan.
  • Coordinate with the child and caregiver on the scheduling and provision of services.
  • Participate in relevant treatment meetings and attend all required trainings.
  • Complete all documentation.
Requirements and Education:
  • Bachelor's degree in a related field and at least two years of related experience OR
  • Master's degree in Social Work, counseling, psych, or a related human service and at least one year of related experience
  • Valid NYS Driver's License required
