Job Title: Nurse Case Manager - Acute Care

Job Summary:

The Nurse Case Manager - Acute Care will be responsible for coordinating patient care across the continuum, ensuring seamless transitions between hospital, post-acute, and community settings. This role requires strong clinical expertise, excellent communication skills, and a passion for delivering high-quality patient care.

Key Responsibilities:
  • Conduct thorough assessments of patients' medical and social needs
  • Develop and implement individualized care plans to achieve optimal patient outcomes
  • Collaborate with healthcare teams to ensure timely and effective transitions between care settings
  • Provide education and support to patients and their families to promote self-management and independence
  • Identify and address potential barriers to care, including social determinants of health
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Associate degree in Nursing required; Bachelor's degree in Nursing or related field preferred
  • Valid, active Registered Nurse (RN) license in the State of employment
  • Minimum 2 years' clinical work experience; 1 year of utilization review and/or case management experience preferred
  • Strong communication and interpersonal skills; ability to work effectively with diverse stakeholders
  • Ability to work autonomously and prioritize tasks effectively
