Travel Nurse Case Manager Job Description

We are seeking a skilled Travel Nurse Case Manager to join our team at Travel Nurse Across America. As a Travel Nurse Case Manager, you will work with a highly skilled team of professionals to advocate for patients, evaluate, plan, provide resources, and facilitate communication with family members.

Key Responsibilities:
  • Evaluate and assess new patients to develop comprehensive care plans.
  • Create and update short and long-term patient care plans to ensure effective care coordination.
  • Educate families and patients about resources and healthcare options to promote informed decision-making.
  • Collaborate with other healthcare professionals, including RNs, doctors, and social workers, to ensure seamless care delivery.
  • Properly document and submit medical records to maintain accurate patient information.
  • Communicate with insurance companies to resolve coverage issues and advocate for patients' needs.
  • Analyze results, monitor variances, identify trends, and recommend actions to management to improve financial outcomes.
Requirements:
  • BLS certification
  • 2 years of recent case management experience

As a Travel Nurse Case Manager, you should be able to stand and walk for long periods, carry up to 10 pounds of weight, and bend, lean, and stoop without difficulty. You should also possess good oral and written communication skills to effectively interact with patients, families, and colleagues.
