Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Healthcare Coordination Specialist. This role is responsible for coordinating referral management functions and ensuring referral centers receive information in a timely and efficient manner.

About the Role

The successful candidate will work closely with multidisciplinary teams to facilitate continuity of patient care. They will be responsible for providing consultation and administrative support for TRICARE beneficiaries, optimizing patient flow by assisting patients through the Central Referral Center, and performing data collection and review to identify areas requiring intensive management.

Key Responsibilities

  • Review specialty care referrals for appropriateness, completeness, and disposition
  • Analyze data collected and prepare reports describing resource utilization patterns
  • Disseminate information to government members of collaborative teams and multidisciplinary hospital committees
  • Maintain and track referral records
  • Participate with referral centers in providing, assessing, and improving customer services and relations
  • Assist in managing communication between beneficiaries, referral centers, internal staff, providers, and network/outside providers
  • Work knowledge of latest versions of office automation software and Composite Health Computer System required

Requirements

  • English Language Requirement: Ability to read, understand, speak, and write English well enough to communicate effectively
  • Education: Successful graduation from an approved National League of Nursing Program
  • Licensure/Registration: Valid and current Colorado State nursing license with no limitations or pending adverse actions
  • Certification: Current Basic Cardiac Life Support certificate and desireable Advanced Cardiac Life Support and Certified Professional in Healthcare Management certification
  • Experience: Minimum of two years of clinical experience and working knowledge of computers, specifically Internet, Microsoft Word, Access, Excel, and Windows
