MDS Coordinator Job Description

We are seeking an experienced MDS Coordinator to join our team at GPAC, a leading search firm specializing in placing quality professionals within multiple industries across the United States.

Position Overview:

The MDS Coordinator will play a critical role in our facility's success by managing the Minimum Data Set (MDS) process in accordance with federal and state regulations. This includes overseeing the completion of MDS assessments in a timely and accurate manner, ensuring compliance with federal and state regulations, and collaborating with the interdisciplinary team to develop individualized care plans based on comprehensive resident assessments.

Key Responsibilities:
  • Oversee the completion of MDS assessments in a timely and accurate manner
  • Ensure compliance with federal and state regulations, as well as facility policies
  • Collaborate with the interdisciplinary team to develop individualized care plans based on comprehensive resident assessments
  • Coordinate and facilitate care plan meetings to ensure resident needs are met
  • Monitor and audit documentation for accuracy, completeness, and compliance with CMS regulations
  • Train and educate staff on MDS processes, regulations, and documentation requirements
  • Optimize facility reimbursement through effective management of MDS data submission and quality reporting
Qualifications:
  • Licensed RN or LPN
  • Minimum of 2 years of experience as an MDS Coordinator or in a related role within a long-term care or skilled nursing facility
  • Comprehensive knowledge of MDS 3.0, RAI process, and care planning
  • Strong understanding of Medicare, Medicaid, and reimbursement processes
  • Excellent attention to detail, organizational, and time-management skills
  • Strong interpersonal skills with the ability to communicate effectively across disciplines
